9:00 a.m. to 4:00 p.m.
New Patients
Please arrive at least 15 minutes prior to your appointment time. This will allow your information to be registered within our systems. Please bring the following items to your first visit:
- Completed new patient forms
- Current driver’s license
- Current health insurance card(s)
- A list of your current medications
- Your pharmacy name and phone number
- Your primary care physician and/or your referring doctor’s name and phone number
If you require special assistance (e.g., translation services), please notify our staff so that we can make accommodations for you.
Cancellations
Please notify us 24 hours prior to your appointment if you need to cancel or reschedule your appointment by contacting our office at (817) 684-2700 during normal business hours.
No Show Appointments
If you do not show up for your scheduled appointment, you may be charged a $25.00 cancellation fee that will be your responsibility (this fee cannot be submitted to your insurance company).
Payments
It is our policy that payment is due at the time of service. Our office diligently works to verify your insurance benefits, but it is still ultimately your responsibility to be familiar with your insurance benefits. If applicable, you are responsible for the proportion or co-pay that your insurance will not cover for your visit and for any procedure(s) that may be performed during the visit.
PLEASE NOTE: insurance verification is not a guarantee of payment from your carrier.
Medication Refills
We have a centralized refill request center to allow more timely responses to your request. Please have your pharmacy fax your medication request to our office. Please call the office if you are requesting pain medication refill as you may need to be re-evaluated.

Our practice is dedicated to maintaining the privacy of your health information. We are required by law to maintain the confidentiality of your health information.
